What the contact form collects

When you ask for a guide or to be matched with a provider, the form collects your first name, email, phone, state, what matters most in your search, the page you came from, any provider you are interested in, and a record of the choices you made. We do not ask for your weight, diagnosis, or medication history here, and we do not include your health-related quiz answers.

Your consent and choices

By submitting, you agree to be contacted about your request and — if you choose to be matched — to have a provider reach you by phone, text, or email. Consent is not a condition of purchase, and you can withdraw it at any time. You can unsubscribe from email whenever you like, and you can ask us to show, correct, or delete the information you gave us. We do not sell your information.

Corrections

If a price, term, or other detail on a page is out of date, tell us and we will check it against the source and fix or remove it.
